What to Expect

Before Your First Visit:

  1. Complete Your Patient Forms Online: You will receive a secure link to complete your intake forms electronically before your appointment.
  2. Upload Relevant Medical Records: If available, please upload prior medical records, lab results, radiology reports, or diabetes logs through the patient portal or send them to us ahead of time via secure email or fax.
  3. Medication List: Be ready to review your current medications and supplements, including dosages. You may upload this in advance or have it on hand during your visit.
  4. Diabetes Patients: If applicable, have your blood glucose logs, meter, or insulin pump data ready to discuss during the appointment.
  5. Technology Check: Ensure you have a stable internet connection and a device (smartphone, tablet, or computer) with a camera and microphone. Test your device before your visit if it’s your first time using telehealth.


For Follow-Up Visits:

  1. Prepare Your Health Logs: Keep your most recent blood glucose readings, blood pressure logs, or symptom trackers accessible for review.
  2. Lab Work: If lab tests are ordered, please complete them at least 7–10 days before your appointment to ensure results are available for discussion.
  3. Updated Medication List: Have an up-to-date list of your medications ready for each visit.
  4. Questions or Concerns: Make a note of any questions or issues you’d like to address during the appointment so we can make the most of our time together.
